Questionnaires and consent forms are due before your appointment. Please make sure to sign up for the patient portal after you make your appointment.
No shows are charged 100% of service fee and cancellations less than 24 hours before appointment time will be charged 50% of the service fee. Payment is due in full before your appointment. Patients 10+ minutes late for an appointment are considered a no show.
